The accuracy difference is HUGE. 7N1 was specifically designed to be a high performance, extremely accurate, cartridge. It uses the same case and bullet diameter as 7.62x54R, but the similarities end there. The 7.62x54R cartridges you buy surplus were manufactured to be used in Soviet machine guns. So they're made to lower tolerances and the bullets weren't designed to be accurate. 7N1 ammo has much higher quality control standards, uses a different bullet design (boat-tailed), and has several other features which significantly improve accuracy.
