For the DVI, is it DVI-A, DVI-D, or both for each PC and the monitor? The DVI to VGA are probably only connecting the analog pins.

Do you have pins C1-C5 on everything?
I gave up on external video switching long ago. The good stuff is expensive, the affordable stuff causes problems (if it works).
I wound up using a USB RS-232 adapter, Synergy, and a small script. Synergy makes one keyboard and mouse the master for everything, the script watches the Synergy log for screen switches then blasts RS-232 data down to the monitor for input switching. Similar switching can be done via IrDA if needed.
